Privacy Policy

With the rise of new communication tools, protecting privacy is essential. We are therefore committed to respecting the confidentiality of the personal data collected. Our staff must protect the privacy of their clients. At Centre Fortitude, all employees sign a confidentiality agreement to guarantee this protection.

Customer data and its protection

The Fortitude Centre complies with provincial privacy laws regarding the collection and protection of confidential information.

Why collect information and how will it be used?

Personal information includes the customer's name, telephone number, and other basic contact and demographic information, as well as data used for evaluation. This data, collected through forms, questionnaires, and website interactions, is used to:

  • Correspondence with the client

  • Directing services

  • Booking appointments

  • Identify clients and associated files

  • Communicating with clients (e.g., appointment changes)

  • Administrative and accounting purposes (invoicing and collection)

  • To facilitate the development and management of an assessment, diagnosis, treatment and continuity of appropriate care.

Consent

Client consent is obtained initially through a preliminary questionnaire and during the first session. By continuing the assessment or intervention, the client agrees to the collection, use, and disclosure of their personal information. Confidential information may be shared for supervisory or consultation purposes within the Centre. Client consent is required for any other disclosure of the client's file.

What customers can expect

Personal information is used, disclosed, and retained only for the intended purposes, with the client's consent. It is stored in a secure environment, and our staff is bound by confidentiality agreements. Disclosure without consent is possible if legally required or permitted. Information may be shared with third-party payers to ensure payment for services and continuity of care, as per an agreement with the client. We ensure the accuracy of the data, which is securely stored in paper and/or electronic format.

Security

We guarantee a high level of confidentiality thanks to the latest technologies. However, no system is infallible, and using the Internet to transmit personal information always carries a risk.

Confidentiality and its limits

Exceptions to psychological confidentiality include:

  • Warn a potential victim if a client plans to cause them harm.

  • Inform the authorities if a client presents an imminent risk of suicide.

  • To provide a client's file upon judicial request.

  • To testify under oath before a court.

  • Answering questions from the Quebec Order of Psychologists during an ethics and professional conduct investigation.

  • Report any abuse or exploitation of a child, an elderly person or a disabled person.

  • Report a case of abuse or exploitation of a client by a healthcare professional.

The customer and his rights

  • Customers have the right to know why their personal information is collected, how it is used, and to whom it is disclosed.

  • Customers can request access to their personal information and fees may apply for processing such access requests.

  • Customers can dispute the accuracy of their information and request changes.

  • Access requests should be addressed to the clinic management.

  • Customers can ask questions about Fortitude Centre's compliance with privacy legislation.

Rights of withdrawal and objection, and the person responsible for the protection of personal information

In accordance with legislation PL64 (Law 25), Dr. Louis Freyd, PsyD/PhD, psychologist and general manager, assumes the role of person responsible for the protection of personal information within the Fortitude Centre.

If you consider yourself a victim of a breach of confidentiality

In the event of an incident concerning the confidentiality of your personal information, please inform our Data Protection Officer, Dr. Louis Freyd, psychologist, using the contact details provided above. We are committed to processing your request as quickly as possible and to rectifying any inappropriate situation promptly.

Complaints Management Policy according to Law 25

Complaints Officer

 

The complaints officer also manages the protection of personal information. They handle all complaints according to the procedure below.

 

Filing a Complaint

Complaints must be submitted in writing with a clear description of the problem.

 

Complaint Receipt

The person in charge acknowledges receipt of the complaint within 7 working days.

 

Complaint Processing

The person in charge begins the analysis of the complaint upon receipt, to be completed within 30 days or more if necessary, with regular communication to the complainant.

 

Complaint Resolution

The person in charge sends a final written and reasoned response, indicating the measures taken.

 

Complaint File

Each complaint is documented with:

  • The plaintiff's written complaint;

  • The results of the analysis and evidence;

  • The final written response to the complainant.

 

The files are confidential, secure and accessible only to the manager via a dedicated complaints register.

 

Effective Date

This policy has been in effect since June 1, 2025.
